The DOT 5800 report should be submitted to the DOT every time there is a hazardous material spill that meets the reporting requirements of the 5800 form. The DOT 5800 form is generated from the Hazardous Material Spill recording form. When adding an incident that involves a Hazardous Material Spill ensure that the Hazardous Material Spill Form is generated. All of the questions on the DOT 5800 are included on the Hazardous Material Spill Form. To generate a DOT 5800, the user needs to go to the Hazardous Material Spill for associated with the incident. Complete all of the appropriate fields and select the Print DOT 5800 button. The report will appear in a new window.
