For each incident number there is the possibility of having five different forms associated with a single incident: Employee Form, Supervisor Form, Safety Form, Security Form and Hazardous Material Spill Form. A system administrator is able to modify the tiles of these forms and determine if each form is available. A Supervisor Form is always generated for an incident. If the other 4 forms are available the user has the ability to determine if all or none of these forms should be generated for this incident. Via the System Functions module, a system administrator can change the form names and determine whether the Employee, Safety, Security and/or Hazardous Material should be available. User access the incident forms by selecting the Incident Number on the Incident Summary Screen. The Choose Recording Form page will appear with hyperlinks to the forms associated with the Incident Number selected.


Quick Steps Example: Editing a Supervisor Form
1.Open the Incident module by selecting the Incident tab in the main menu.  
2. On the vertical menu select Summary  
3.Find the incident in the incident summary.  
4.Select the incident number.  
5.The Choose Recording Form page will appear.  
6.Select the Supervisor Link.  
7.Enter the appropriate information  
8.Select the Save button.  
9.If all required fields are complete and all data is in a valid format, the Successful Save message will appear.
